Thank you for choosing Selma Medical Associates, Inc. to provide you with your mental health needs. Please complete the forms to the right and return them to us either by email to firstname.lastname@example.org, or at any reception desk at least 3 days in advance of your appointment. This will allow important information to be entered into your medical records before your first telemedicine appointment.
If you know in advance that you cannot keep your telemedicine appointment we would appreciate your call to cancel as soon as you know you will not be able to come. If the appointment is not cancelled with at least 24 hours notice, you may be billed a fee of $50.00.
If you have an insurance we participate with we will file your insurance claims for you. Please send or bring your insurance cards with your completed forms.
You will need to decide whether communication by you is either by Zoom or Whattsapp.
